1. What medical forms do we need to complete?

If your child is entering Pre-Kindergarten, please contact the school office for the necessary paperwork. If your child is entering K-8, below are the medical records needed in order for your child to start school. Please upload to FACTS Application & Enrollment or turn in a hard copy to the school office prior to the start of school. Note that no philosophical or religious immunization exemptions are accepted.

2.  I just submitted my application. What’s next?

Applications will be considered on a rolling basis as space allows. Once your application is reviewed and accepted, you will receive an offer of admission via email. It is important to look out for this email as it has the link to begin the Enrollment process which is Step 2. (Be sure to check Spam). Click the personalized link and proceed to complete the information required for Enrollment through your FACTS account.

3. How do I sign up for the CARES After School Program?

The sign up form for Cares is sent home with the students during the first week of school.
